Charge Floor Nurse FT PM Shift: 7:00 PM to 7:00 AM (RN or LPN) – Full-Time Desert Highlands Care Center
Position Summary:
We are seeking a compassionate, dependable, and clinically skilled Charge Floor Nurse (RN or LPN) to provide exceptional overnight nursing care within our skilled nursing facility. The Charge Nurse is responsible for supervising nursing staff, coordinating resident care, ensuring regulatory compliance, and promoting a safe and supportive environment for residents during the PM shift.
The ideal candidate demonstrates strong leadership, critical thinking, and communication skills while maintaining high-quality resident-centered care in a fast-paced healthcare setting.
Key Responsibilities:
Provide direct nursing care and oversee resident care services during assigned shift
Supervise and support CNAs and nursing staff to ensure quality resident care and workflow efficiency.
Administer medications, treatments, and physician orders accurately and timely
Monitor resident conditions and promptly communicate changes to physicians, families, and leadership.
Complete nursing assessments, documentation, and charting in accordance with facility policies and regulations
Ensure compliance with CMS guidelines, state regulations, infection control, and facility standards.
Respond appropriately to emergencies, incidents, and resident concerns
Assist with admissions, discharges, and transfers as needed
Promote a positive team environment through leadership, professionalism, and collaboration.
Participate in shift reports and ensure continuity of care between nursing teams
Maintain resident dignity, confidentiality, and safety always.
Qualifications:
Current RN or LPN license (state required)
Minimum 1–2 years of nursing experience preferred in skilled nursing or long-term care.
Strong knowledge of nursing practices, medication administration, and regulatory compliance.
Experience with EMR/EHR systems (PointClickCare preferred)
Excellent leadership, organizational, and communication skills
Ability to multitask and work effectively in a fast-paced environment
Ability to work independently and as part of a healthcare team
Schedule:
Full-Time
12-hour overnight shifts
May include weekends and holidays as scheduled
Compensation:
LPN Starting Rate is Based on Years of Experience
RN Starting Rate is $37

Admissions Nurse FT Shift: 1:00 PM to 10:00 PM (RN or LPN) – Full-Time – Skilled Nursing Facility – Monday through Friday

We are seeking a compassionate, organized, and clinically skilled Admissions Nurse (RN or LPN) to oversee and coordinate the admissions process within our skilled nursing facility. This role plays a vital part in ensuring smooth resident transitions, supporting census growth, and creating a positive first impression for residents and families.
The ideal candidate combines strong clinical judgment, communication skills, and attention to detail to ensure safe admissions, regulatory compliance, and exceptional customer service.
Key Responsibilities:
Coordinate and manage all clinical aspects of admissions and re-admissions
Review hospital referrals, clinical documentation, and insurance requirements for admission appropriateness
Complete admission assessments, physician orders, and required documentation accurately and timely
Collaborate with hospital case managers, physicians, discharge planners, and interdisciplinary teams to facilitate admissions
Ensure compliance with CMS guidelines, state regulations, and facility admission policies
Provide residents and families with a welcoming and smooth admission experience
Complete medication reconciliation, initiate treatments, and assist with care plan development
Work closely with Admissions and Marketing teams to support census development goals
Maintain accurate tracking logs for referrals, admissions, denials, and conversion metrics
Participating in weekend admissions coverage and on-call support as needed
Communicate effectively with nursing staff and leadership regarding new admissions and clinical updates
Qualifications:
Current RN or LPN license (state required)
Minimum 1–2 years of experience in skilled nursing, acute care, admissions, or case management preferred
Strong knowledge of clinical documentation, Medicare, Medicaid, and managed care processes
Experience with EMR/EHR systems (PointClickCare preferred)
Excellent communication, customer service, organizational, and multitasking skills
Ability to work effectively in a fast-paced, high-volume admissions environment
Strong critical thinking and problem-solving abilities
Schedule:
Full-Time
Monday through Friday
May include rotating weekends or on-call admissions support
Compensation:
LPN Starting Rate is $35
RN Starting Rate is Based on Experience
