Community Care, Inc. is looking for a compassionate and dedicated PTA to provide home health care to our clients in the Westmoreland and Fayette County area. Provide physical therapy treatment plans as established by the supervising Physical Therapist

Provide patient education and instruction in home exercise programs

Monitor, document, and report patient progress and any changes in condition

Support patient in achieving functional goals that enhance safety and independence

Collaborate with a multidisciplinary team to ensure continuity of care

Document visits and patient progress accurately and timely
-
Current Physical Therapist Assistant (PTA) license in the state of Pennsylvania.

Graduate of an accredited Physical Therapist Assistant program

Minimum 1 year of clinical experience (Home Health experience preferred)

CPR certification (required)

Reliable transportation and a valid driver's license
