Our team includes Registered Nurses, Licensed Vocational Nurses, Physical Therapists, Occupational Therapists, Social Workers, and Speech Therapists, supporting veterans and active duty families, seniors, and children and adults with special needs.
Position Summary
Dependable Nursing is looking for a Physical Therapist to cover home health cases throughout San Diego County. Our current caseload runs approximately 50% North County (currently supported by our existing team), 25% Central/South County, and 25% East County, so we need someone centrally located who can efficiently reach Central, South, and East County territory. This role owns the full scope of PT-driven OASIS assessments and, in the early stages of building out the territory, will also carry a hands-on visit caseload. As census grows, Physical Therapist Assistants (PTAs) will be added to take on routine visit care, allowing the PT to shift focus toward evaluations, OASIS documentation, plans of care, and PTA oversight.
Position Description
- Complete OASIS assessments for Start of Care (SOC), Resumption of Care, Recertification, Transfer, and Discharge.
- Evaluate patients and develop individualized plans of care based on physician orders and patient needs.
- Provide direct, hands-on physical therapy treatment visits across the assigned territory, particularly during initial caseload buildout.
- Identify and document maintenance therapy needs, including completion of OASIS for maintenance programs.
- Delegate and oversee routine visit-based care to Physical Therapist Assistants (PTAs) as territory and census grow.
- Collaborate with the interdisciplinary care team (RN, PTA, OT, ST, SW) to coordinate patient care.
- Maintain timely, accurate clinical documentation in compliance with Medicare, Medi-Cal, and agency standards.
- Travel throughout San Diego County to provide in-home patient visits.
Requirements
- Must have a current CA Physical Therapist (PT) license.
- Must have current CPR certification.
- Working knowledge of OASIS for SOC, Resumption of Care, Recertification, Transfer, and Discharge.
- Understands Medicare rules and regulations
- Experience with maintenance therapy programs and related OASIS documentation preferred.
- Home health experience preferred.
- Access to reliable transportation and ability to travel throughout San Diego County.
- Must have the ability to obtain a base pass with a Real ID or US Passport.
Pay: $100,000.00 - $115,000.00 per year
