The STAR Health Center at SUNY Downstate Health Sciences University is a comprehensive ambulatory care program caring for the most vulnerable populations in Central Brooklyn, including people with HIV and HCV, immigrants, LGBTQ+ populations, reentrants, and people with mental health and substance use disorders. STAR's Supportive Counseling Project is seeking a Patient Navigator to provide support and health education to patients. Patient Navigators at Downstate's STAR Health Center are essential members of our interdisciplinary team and help individuals navigate the healthcare and public benefits systems. This is a great opportunity for an undergraduate or graduate school student seeking a part-time position that will provide them with a valuable direct service experience in a health/mental health setting. Patient Navigators address barriers to care such as food insecurity, LGBTQ+ stigma, homelessness, substance use dependency and/or mental health issues by conducting home visits, arranging transportation, accompanying patients to appointments, and providing health promotion and adherence support.
REQUIREMENTS: A.A./A.S. degree or equivalent college credits with internship or some experience working in a community-based agency, health care or mental health organization. College and graduate school students are encouraged to apply. May consider high school diploma/GED with 2-3 years of experience working in a community-based agency or healthcare organization. Effective computer, verbal, organizational and interpersonal skills required. Bilingual Creole or Spanish speaking is an asset. Outreach and community-based experience preferred.
SALARY RANGE: $25,000 to $33,000 (18.75 to 22.5 hours per week) based on education and experience. This position offers comprehensive benefits for employees working half-time (18.75 hours) or more.
