Job Description:
We are currently seeking a dedicated and compassionate Physical Therapist (Per Diem) to join our home health team in Lynn and Dorchester, MA. This is a flexible, per-visit opportunity ideal for clinicians who enjoy working independently and providing high-quality care in patients’ homes.
Responsibilities:
Provide physical therapy services to patients in their homes
Evaluate patient needs and develop individualized treatment plans
Complete accurate and timely documentation, including OASIS assessments
Educate patients and families on treatment plans and progress
Collaborate with the interdisciplinary care team to ensure optimal patient outcomes
Qualifications:
Active Physical Therapist (PT) license in Massachusetts
CPR certification (current)
OASIS experience required
Home health experience preferred
Reliable transportation and willingness to travel within assigned areas
Strong communication and documentation skills
English proficiency required
