Community Concierge Cares (C3Cares) is on a mission to provide exceptional care to the communities who need it the most. We are seeking an experienced, compassionate, and mission-driven Lead Family Nurse Practitioner to provide high-quality primary care and serve as a clinical leader for our growing healthcare team.
This is more than a traditional primary care role. As our Lead Family Nurse Practitioner, you will spend the majority of your time providing direct patient care while also serving as a clinical mentor and resource for fellow providers and care team members. You will help foster clinical excellence, support evidence-based practice, and ensure patients receive exceptional, coordinated care.

Position Overview
The Lead Family Nurse Practitioner provides comprehensive primary care services to patients across the lifespan while serving as a clinical leader within the organization.
This role is primarily focused on direct patient care, including preventive services, chronic disease management, acute care, care coordination, and patient education. The Lead Family Nurse Practitioner will also support clinical quality initiatives, mentor providers and staff, and help ensure the delivery of consistent, high-quality care across C3Cares programs.
Key Responsibilities
Direct Patient Care
· Provide comprehensive primary care services for children, adolescents, adults, and older adults.
· Diagnose, treat, and manage acute and chronic medical conditions.
· Conduct physical examinations and health assessments.
· Order, interpret, and evaluate diagnostic tests and laboratory results.
· Develop and implement evidence-based treatment plans.
· Prescribe medications and therapies in accordance with DC regulations and clinical best practices.
· Provide preventive care, screenings, immunizations, and wellness counseling.
· Deliver patient-centered education and support shared decision-making.
· Utilize both telehealth and in-person visits to improve access and continuity of care.
Chronic Disease & Population Health Management
· Manage patients with complex medical and social needs.
· Support care plans for patients with diabetes, hypertension, asthma, COPD, obesity, behavioral health conditions, and other chronic illnesses.
· Identify and address care gaps through preventive and population health initiatives.
· Collaborate with care coordinators, community health workers, behavioral health providers, and specialists to improve patient outcomes.
· Address social determinants of health that impact patient well-being.
Clinical Leadership
· Serve as a clinical mentor and resource for nurse practitioners and care team members.
· Promote evidence-based practice and clinical excellence.
· Participate in case reviews, care planning, and interdisciplinary team discussions.
· Support onboarding and clinical development of providers and staff.
· Foster a culture of compassionate, patient-centered care.
· Assist with implementation of clinical protocols, quality measures, and best practices.
Quality & Patient Safety
· Maintain accurate and timely documentation in the electronic health record.
· Support quality improvement initiatives focused on patient outcomes and access to care.
· Monitor and promote adherence to evidence-based clinical guidelines.
· Ensure compliance with regulatory and professional standards of practice.
· Participate in chart reviews, clinical audits, and performance improvement activities as appropriate.
Pay: From $145,000.00 per year
