We are seeking a hard-working and compassionate Physical Therapist to join our healthcare team. The ideal candidate will be responsible for assessing and implementing individualized care plans for patients in the home health environment. This role requires a strong understanding of medical terminology, physiology, and patient customer service to provide effective treatment and support to patients recovering from injuries or managing chronic conditions.
Responsibilities
- Develop and implement personalized care plans based on patient assessments and goals.
- Provide direct patient care through therapeutic exercises, manual therapy techniques, and modalities.
- Educate patients and their families about treatment plans, exercises, and self-care strategies.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate documentation of patient progress and treatment outcomes in compliance with medical standards.
- Stay updated on the latest research and best practices in physical therapy to enhance patient care.
- Conduct evaluations of patients’ physical abilities and limitations to determine appropriate interventions.
- Provide inpatient therapy services as needed, ensuring a safe environment for all patients.
Qualifications
- Master's or Doctorate in Physical Therapy degree from an accredited program.
- Valid state licensure or eligibility for licensure as a Physical Therapist.
- Strong knowledge of physiology and medical terminology relevant to physical therapy practice.
- Experience in inpatient settings or home community care is preferred but not required.
- Familiarity with developing care plans tailored to individual patient needs.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work collaboratively within a multidisciplinary team environment.
- Commitment to providing high-quality patient-centered care.
- Strong Interpersonal skills.
- Must have reliable transportation.
