What You’ll Do
- Provide skilled nursing visits in patients’ homes
- Assess and reassess patient needs and treatment plans
- Coordinate care with physicians and clinical teams
- Educate patients and families
- Administer and monitor medications
- Support medical and behavioral health treatment goals
- Develop individualized care and safety plans
- Promote independence and improved patient outcomes
Qualifications
- Active Massachusetts RN License
- Current CPR Certification
- Valid Driver’s License
- Reliable vehicle with active insurance
- Strong communication and clinical assessment skills
- Spanish Speaking a plus
