Position Details
- Employment Type: Per Diem / PRN
- Location/Coverage Area: North Orange County (including Cypress, La Palma, Buena Park, Cerritos, Seal Beach, Rossmoor)
- Compensation: Competitive Per-Visit Rates (Inquire for detailed rate sheet for Routine, Oasis Start of Care, Resumption of Care, and Discharge visits).
Key Responsibilities
- Conduct initial assessments (OASIS Start of Care), routine visits, re-evaluations, and discharges in the patient’s home environment.
- Develop, implement, and update a patient-centered Plan of Care (POC) in collaboration with physicians and the interdisciplinary team.
- Provide skilled nursing care, including wound care/wound vacs, medication reconciliation, IV therapy, lab draws, and chronic disease management education.
- Complete clinical documentation accurately and timely via our Electronic Medical Record (EMR) system.
- Educate patients and their family caregivers on safety, care techniques, and symptom management.
Qualifications & Requirements
- Licensure: Active, unencumbered Registered Nurse (RN) license in the State of California.
- Experience: Minimum of 1 year of clinical RN experience required. Prior home health experience is highly preferred.
- EMR Skills: Knowledge of Kinnser/WellSky EMR is strongly preferred (experience with OASIS documentation is a major plus).
- Certifications: Current BLS/CPR certification.
- Transportation: Must have a reliable vehicle, a valid CA driver’s license, and active auto insurance for traveling to patient homes.
- Skills: Strong clinical judgment, excellent communication, and the ability to work independently in the field.

Pay: $70.00 - $90.00 per hour
