Job Summary
We are seeking a dedicated and compassionate Home Care Nurse-LPN (Licensed Practical Nurse) to join our dynamic healthcare team. Duties
- Deliver direct patient care including assessments, vital signs, medication administration, and wound care within the home setting
- Perform specialized procedures such as IV infusion, catheterization, tube feeding, and sterile dressing changes with precision and adherence to safety protocols
- Conduct comprehensive patient evaluations, including physical examinations and physiological assessments to inform care plans
- Manage documentation accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems, ensuring compliance with HIPAA regulations
- Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of services for long-term or post-acute care patients
- Provide education on disease management, medication use, nutrition, and health coaching tailored to individual needs
- Respond promptly to urgent situations such as acute pain episodes or infectious disease concerns through triage skills and critical thinking
Skills
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for seamless documentation
- Extensive experience in various healthcare settings including home & community care, hospice, nursing homes, hospitals (ICU, Emergency Room), assisted living facilities, and skilled nursing environments
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding for procedures and diagnoses
- Skilled in performing advanced clinical procedures such as venipuncture, phlebotomy, sterile technique, airway management, ventilator support and spinal taps
- Experience working with diverse patient populations including pediatrics (neonatal ventilator), geriatrics (memory care & dementia), individuals with developmental disabilities or disabilities requiring specialized care
- Ability to handle complex cases involving acute pain management, infectious disease control, trauma (Level I & II Trauma Center experience), and post-surgical recovery including suturing and dermal fillers when applicable
- Familiarity with medical imaging processes and diagnostic evaluation techniques used in patient assessment
- Excellent patient care skills combined with strong communication abilities for effective health coaching and family education
- Knowledge of HIPAA compliance standards to ensure patient confidentiality at all times
