Summary: Our Spiritual Care and Bereavement Coordinator is responsible for the provision of spiritual care services, along with planning, implementing and maintaining a bereavement program to meet the needs of our patients, families, and caregivers.

The position requires travel throughout our Tri-County service area, including Macomb, Oakland, and Wayne counties. You will be responsible for driving to and from patient homes and facilities within this coverage area to deliver the support necessary.
Essential Job Functions:
1. Conducts comprehensive spiritual assessments for patients and families/caregivers enrolled in the hospice program, ensuring services are initiated in a timely manner (within five days of admission).
2. Provides ongoing spiritual and pastoral support in accordance with the patient’s and family’s wishes and evolving needs.
3. Develops and implements an individualized spiritual/pastoral plan of care that:
a. Demonstrates collaboration with local clergy when desired by the patient and family.
b. Delivers spiritual support as identified and defined by the patient and family.
4. Facilitates religious services, including prayer, worship, and rituals, as appropriate to the patient’s beliefs and preferences.
5. Creates a supportive environment for patients and families of diverse philosophies and faith traditions to share and explore their thoughts, feelings, beliefs, and values.
6. Manages the bereavement program, including follow-up phone calls and/or visits, and coordinates monthly bereavement mailings.
7. Provides consultation, education, and support to the interdisciplinary team regarding spiritual care and bereavement services.
8. Officiates funeral or memorial services upon request.
9. Develops and enhances bereavement programs and materials as needed to meet the needs of families and the community.
10. Leads and presents the spiritual care and bereavement components during interdisciplinary team meetings.
Qualifications:
- Master’s degree in theology, religious studies, divinity or pastoral care/studies. Degree must be from an accredited graduate school of theology or seminary.
- One unit of CPE (Clinical Pastoral Education.) Two to four units preferred.
- Demonstrates clinical experience in areas of grief and loss, as well as, individual and family counseling.
- Ability to work as member of interdisciplinary team/group.
- Comfort in an interfaith setting.
- Ability to accept different lifestyles, cultures, beliefs, and values.
- Ability to network with community clergy and congregations.
- Knowledge of and commitment to hospice philosophy of care.
- Must have proficient computer skills including Microsoft Word and Excel.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Preferred - Previous experience working as a chaplain in a hospice setting, hospital and/or long term care facility.
