Position Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care to individuals with intellectual and developmental disabilities (IDD) in a structured day program setting. The LPN plays a key role in supporting participants’ medical needs while promoting independence, dignity, and overall well-being. This position requires collaboration with an interdisciplinary team to ensure the delivery of person-centered, holistic care.
Key Responsibilities
- Provide direct nursing care and health-related support in accordance with established care plans
- Administer medications and treatments as prescribed, ensuring compliance with all regulatory standards
- Monitor and document participants’ health status, reporting any changes promptly
- Maintain accurate, detailed, and timely clinical documentation
- Assist with medical evaluations, appointments, and coordination of care
- Support participants with activities of daily living (ADLs) as needed
- Promote independence and encourage safe participation in program and community-based activities
- Advocate for participant rights while ensuring a safe and respectful environment
- Collaborate with interdisciplinary team members to develop, implement, and update individualized care plans
- Provide health education and guidance to participants and their families
- Respond effectively to medical emergencies and ensure proper follow-up care
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of employment, in good standing
- Prior experience working with individuals with intellectual and developmental disabilities preferred
- Working knowledge of conditions common in IDD populations (e.g., epilepsy, autism spectrum disorder)
- Current CPR/First Aid certification, or willingness to obtain
- Strong clinical, communication, and organizational skills
- Ability to work both independently and as part of a multidisciplinary team

Pay: $30.00 - $40.00 per hour
