Responsibilities:
- Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs.
- Collaborate with the interdisciplinary team to develop, implement, and evaluate personalized care plans.
- Provide skilled nursing interventions including wound care, medication management, IV therapy, symptom/pain control, and patient/family education.
- Deliver emotional support and education to patients and caregivers based on individual needs.
- Supervise LPNs/LVNs and home health aides while coordinating care activities and documentation.
- Respond to urgent needs, participate in admissions/on-call, and adjust plans based on ongoing monitoring.
- Ensure accurate, compliant clinical records and high-quality, compassionate care delivery.
Qualifications:

Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.

One (1) year experience as a RN.
