NOW HIRING INTAKE SPECIALIST!
Responsibilities:
- Acts as the first point of contact for incoming requests and supports them in completing intake forms and documents.
- Responsible for coordinating the intake, scheduling and care delivery process for people with developmental disabilities.
- Conduct interviews with people to compile intake information.
- Make referrals to service providers based on people’s needs and assessment.
Qualifications:
- Bachelor’s degree in health and/or Human Services PLUS (1) yr experience working with people w/ developmental disabilities OR (1) yr experience as service coordinator/case manager OR
- Associate’s degree in health and/or Human Services PLUS (3) yrs experience working with people w/ developmental disabilities OR (3) yrs experience as service coordinator/case manager OR
- High school diploma PLUS (6) yrs experience working with people w/ developmental disabilities OR (6) yrs experience as service coordinator/case manager

Additional Information:
- 40-hour work week (Monday – Friday)
- $23.00- $25.00 per hour (Based on experience)
