Modern Regenerative Health is hiring a Family Nurse Practitioner (FNP) for a Full-Time or Part-Time Primary Care Provider role focused on adult and geriatric patients. Key Responsibilities
- Provide comprehensive primary care services to adult and geriatric patients
- Conduct physical exams, health assessments, and Medicare Annual Wellness Visits
- Diagnose and manage acute and chronic medical conditions
- Develop and manage individualized treatment plans
- Order, interpret, and review labs and diagnostic tests
- Prescribe medications in accordance with state and federal regulations
- Provide patient education on preventive care, lifestyle, and disease management
- Collaborate with clinical staff and multidisciplinary providers
- Maintain accurate and timely electronic medical records (EMR/EHR)
Required Qualifications
- Active Family Nurse Practitioner (FNP) license in New Hampshire (or eligible)
- Board-certified Nurse Practitioner (AANP or ANCC)
- Active DEA license (or ability to obtain)
- Experience in primary care, internal medicine, adult medicine, or geriatrics preferred
- Strong clinical judgment and patient communication skills
- Ability to work independently and collaboratively
