Essential Job Duties:
- Provide care to clients in accordance with the individualized Plan of Care developed by the supervising nurse.
- Assist clients with transfers, ambulation, and prescribed exercises as instructed by the supervising nurse.
- Prepare modified diets as directed by the care plan.
- Observe and report changes in clients' vital signs, skin condition, or symptoms to the supervising nurse.
- Remind clients to take prescribed medications as instructed.
- Perform light household tasks necessary for a safe and comfortable home environment, such as cleaning and grocery shopping.
- Report any changes in the client's physical status promptly to the supervising nurse or agency.
- Submit required annual compliance documents (e.g., TB questionnaire).
- Complete 12 hours of mandatory annual compliance training as required by the New York Board of Nursing.

- Ability to communicate clearly and effectively with clients, families, and care teams.
- Commitment to following infection-control practices and maintaining client safety.

Qualifications:
- Active New York HHA Certification
- Must be at least 18 years of age and able to read and write in English; bilingual candidates are encouraged to apply.
- Must have a physical examination meeting New York State Department of Consumer Affairs requirements for home health care workers.
- Demonstrate compassion, reliability, and professionalism.
