Job Overview
We are seeking a dedicated and organized LVN as Care Coordinator/ case manager in home health. The ideal candidate will play a pivotal role in ensuring that patients receive comprehensive and quality care by coordinating their care between staff and physician. This position requires a good communication and customer service skills, compassionate and understanding . The Care Coordinator will obtain reports from patients field staff ,facilitate communication between patients, healthcare providers, and to enhance the overall patient experience.
Responsibilities
- Serve as the primary point of contact for patients, addressing inquiries and providing information regarding services.
- Coordinate field staff schedules, ensuring patients are visited and services are provided as ordered
- Ensure accurate patient visits records and documentation in compliance with healthcare regulations.
- Perform insurance verification and authorization processes to facilitate patient care.
- Ensure implementation of individualized care plans tailored to patient needs.
- Perform administrative support, including checking faxes, orders, managing correspondence, filing, and data entry.
- Ensure a smooth flow of communication between patients, field staff, and administrative teams.
Qualifications
- Previous experience in home health care setting or case management is preferred.
- At least 2 years eperience in acute care setting, skilled nursing facility or home health
- Experience in customer service preferred
- Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent interpersonal skills to foster positive relationships with patients and staff members.
- Proficient in office administration tasks, including scheduling, record keeping, and quality management.

Pay: $35.00 - $38.00 per hour
