Position Description:
Provide nutritional services to patients of the agency, under the direction and written orders of a physician. The dietitian works within the scope of practice as defined by the NYS Education Department for dietitians. The dietitian practices his/her duties in a manner that is compliant with federal, state, and local codes, rules regulations, and law.
Essential Duties and Responsibilities:
- Within one week of patient referral the Dietician evaluates the patient's dietary needs, cultural background and food habits.
- Performs nutritional counseling upon the written orders of a physician.
Develops a treatment plan based upon patient evaluation and physician orders, designed to optimize and maximize the patient’s nutritional status.
- Determines that the basic dietary needs of the patients are being met.
- Provides necessary guidance regarding the implementation of the diet and the selection, storage and preparation of food.
- Documents on designated records all evaluations, re-evaluations, progress notes, consultations, conferences, and discharge summaries, within the time frames established by policy and procedure. Submits initial assessment and Plan of Care within 72 hours of visit, and ongoing documentation on a weekly basis.
- Participates with other personnel in the formulation of patients' total Plan of Care, and confers with the other team members after the initial assessment visit and every two weeks thereafter, or. more frequently, as needed.
- Consults with the physician as necessary regarding the nutritional plan of care and patient progress.
Other Duties:
- Assists in the development of nutritional policies and procedures.
- Assists in the instruction of agency staff as needed.
Must complete timely and accurate documentation as evidenced by clinical record audits and the appropriate submission of required documentation.
Must have the ability to educate as evidenced by the patient’s and family’s increased knowledge of disease process and treatment, evaluated in supervisory visits and chart reviews.
- Performs other appropriate duties, as needed.
Working Conditions:
- Home Care field work involves increased risk while driving during inclement weather to and from patient’s home and other work assignments.
- Some irregular hours may occur and adjustments made to work assignment for the welfare of the patient.
- Increased exposure to infectious diseases; must follow established protocols.
Qualifications:
· Graduate of an accredited program in nutritional therapy; registered or eligible for registration by the American Dietetic Association or has the equivalent of such training and experience.
· At least one year of clinical experience preferred;
· Must understand the principles, techniques and practices of nutritional therapy.
· Understanding of the pathological conditions, socioeconomic factors, and cultural background and their effects on the patient’s food habits and nutritional status also required.
- A high degree of confidentiality is required.
Pay: $80.00 per hour
