Merit Home Health Services in is seeking for a compassionate, dependable skill licensed Vocational nurse to join our Home Health team.
we are looking for a dedicated clinician who enjoy working independently and providing quality care to patient in their home
Responsibilities:
As intermittent nurse, you will be essential in delivering high-quality patient care in various settings at the patient home.
Provide skilled nursing visit to visit under the supervision of the registered nurse and according to the physician plan of care.
Perform routine nursing assessment and report significant changes in patient condition to the RN and physician.
Administer medication and treatment as order by the physician
Provide wound care, catheterization, ostomy care, wound vac, nephrostomy tube care, and other skilled nursing procedures within LVN scope of practice
Monitor vital signs and access patient response to treatment.
Reinforce teaching regarding medication disease process safety precautions and infection prevention
Assist with blood glucose monitoring and diabetic education.
Document patient care accurately and timely in the agency electronic medical record.
Communicate effectively with patient, caregiver and physician and interdisciplinary team members.
participate in care conference and coordination activities.
Good documentation while adhering to HIPAA regulations and maintaining accurate documentation.
Qualification:
Current California area license in good standing.
Minimum one year of nursing experience home health experience preferred.
knowledge of wound care and chronic disease management, strong observation. communication, and organizational skills.
Ability to recognize and report changes in patient condition promptly.
Ability to work independently and manage assigned patient visit effectively
Valid driver license, reliable transportation, and current auto insurance

San Bernardino area, Fontana, Colton, Victorville, Perris Ca, Moreno Valley, Hemet, Temecula, Ontario CA, Rancho Cucamonga, Upland, Yucaipa, Beaumont, Riverside, and High Desert.
