Job Summary

Responsibilities
- Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and transfer methods
- Monitor and record vital signs, medication administration, and changes in patient condition using paper or electronic charting systems
- Support patients with complex medical needs such as nurse delegated tasks
- Collaborate with interdisciplinary teams to coordinate care plans, ensuring compliance with HIPAA regulations and maintaining patient confidentiality
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including behavioral health support and safety measures
- Assist with medical procedures such as phlebotomy or basic life support (BLS) when trained and certified
- Educate patients and family members on health management, medication adherence, and safety protocols within the home environment
Experience
- Prior experience working as a CNA in home care settings or long-term care facilities is highly preferred. This position is not open the HCA, NAR, or HHA applicants.
- Demonstrated knowledge of geriatrics, senior care, hospice care, or behavioral health environments
- Experience working with individuals with developmental disabilities or special needs is advantageous
- Certification in CPR and basic life support (BLS) is mandatory along with Food Handlers
- Strong communication skills to effectively engage with patients, families, and healthcare teams while demonstrating compassion and professionalism
- You must be authorized to work, be able to pass a background check, and have at least two verifiable references (non-family)

Pay: $25.00 per hour
Expected hours: 36.0 – 40.0 per week
