Job Description
Our Primary Care team is looking for an exceptional nurse to help us manage and coordinate care for our high-acuity patients. Our nurses enjoy strong relationships with patients, collaborative work environments and a deep sense of belonging and appreciation. Nurses are responsible for carrying out a variety of healthcare services, including:
- Medication management;
- Coordinating care between outside providers and hospital systems;
- Disease management;
- Wound care;
- Recording vitals;
- Developing Plans of Care;
- Phlebotomy;
- Hydration Therapy and more!

Qualifications:
