A 288-bed acute care hospital in Tallahassee is seeking a Registered Nurse Case Manager to join its care coordination team. The facility provides 24/7 emergency services and a wide range of specialty programs, including Behavioral Health, Neonatal Intensive Care, Stroke Services, Heart & Vascular care, and a Chest Pain Center with PCI-Resuscitation capabilities.
This role focuses on supporting effective, efficient, and patient-centered care across the continuum by coordinating interdisciplinary plans of care and ensuring appropriate utilization of clinical resources. The RN Case Manager evaluates medical necessity, admission status, level of care, and ongoing resource needs while working closely with physicians and the care team to support optimal patient outcomes.
Key responsibilities include completing comprehensive psychosocial and clinical assessments, developing individualized case management plans, and facilitating interdisciplinary meetings to support care coordination. The role also involves utilization review, documentation of medical necessity using approved criteria, and communication with third-party payors.
A strong emphasis is placed on discharge planning, identifying barriers to patient throughput, and connecting patients — including those from underserved populations — with appropriate community resources and post-acute services. This position also tracks variances in care, identifies trends, and contributes to process improvement initiatives across the organization.
Qualifications:
Minimum of 3 years of recent acute care Case Management experience required (within the last year preferred) OR equivalent recent acute care nursing experience in Med-Surg, Telemetry, Neuro, ICU, PCU, or ED
Associate Degree in Nursing or Nursing Diploma required
Bachelor of Science in Nursing preferred
Active Florida RN license or compact RN license required (Florida license required within 90 days if compact held)
Advanced Practice Registered Nurse (APRN) license may be accepted if current and compliant
Certification in Case Management or Utilization Review preferred
