Responsibilities
Coordinate and facilitate client intake processes by gathering comprehensive medical histories and social information.
Develop and implement individualized care plans in collaboration with healthcare teams, ensuring alignment with patient needs and preferences.
Manage discharge planning activities by coordinating with hospitals, social services, and community resources to ensure smooth transitions from hospital to home.
Utilize EMR (Electronic Medical Record) systems and EHR (Electronic Health Record) systems to document client information accurately and maintain up-to-date records.
Communicate effectively with clients, families, healthcare providers, and social services to address concerns, provide updates, and ensure quality care delivery.
Conduct utilization review and management to optimize resource use while maintaining high standards of patient care.
Work with individuals from diverse cultural backgrounds, demonstrating sensitivity and adaptability in all interactions.
Requirements
Minimum of critical care experience or hospital experience; ICU or emergency medicine background preferred.
Strong nursing knowledge combined with familiarity with CPT coding, ICD-9/10 coding systems for accurate documentation and billing processes.
Experience in hospice care, acute care settings, or Level I/II trauma centers is highly desirable.
Proficiency in Microsoft Office applications along with EMR/EHR systems to manage patient data efficiently.
Solid understanding of HIPAA regulations, health insurance processes, medical terminology, discharge planning, and managed care principles.
Excellent client communication skills with the ability to work effectively with individuals from diverse backgrounds; bilingual abilities are a plus.
Experience working in pediatric or behavioral health settings is advantageous; PICU or OR experience is a bonus.
Knowledge of utilization review and management practices within hospital or managed care environments.
Demonstrated ability to coordinate case management activities while maintaining attention to detail and organizational skills.
