Schedule: Monday - Friday 8:00 am - 4:30 pm
Description : provide the following:
- Community Support Services
- Counseling
- Case management
- Crisis Intervention
- On-going support to adults with mental health issues at Transitions
- Maintain accurate records regarding all services provided and complete such documentation in an accurate and timely manner
- Responsible for meeting and/or exceeding billing expectation
Requirements:
- Bachelor's degree in human/social services
- At least 1 year of experience in the field is required
- Must have a valid driver's license
- Must have excellent communication and organizational skills
- Sensitive, patient, and responsive.
