POSITION SUMMARY
The Casa Q Safe House Case Manager provides comprehensive, trauma-informed, and youth-centered case management services to LGBTQ+ and BIPOC youth residing in the Casa Q Safe House.
The Case Manager works directly with youth to identify their individual strengths, needs, goals, and barriers while supporting each young person in developing the skills, resources, and connections necessary to increase safety, stability, independence, and overall well-being.
The Case Manager is responsible for individualized goal planning, life-skills development, educational planning, safety planning, care coordination, referrals, documentation, and ongoing assessment of client progress. This position also plays a key role in the Safe House admissions process, including reviewing referrals, communicating with referral sources, conducting pre-admission interviews, and completing youth intakes.
The Case Manager is expected to maintain a regular presence at the Safe House and have flexibility to work evenings in order to engage directly with youth, facilitate programming, conduct individual meetings, and respond to the needs of the program.
ESSENTIAL DUTIES AND RESPONSIBILITIES1. Direct Youth Case Management
- Establish and maintain supportive, professional, and trusting relationships with youth.
- Provide individualized case management services based on each youth's strengths, needs, goals, and circumstances.
- Complete assessments and identify areas requiring support or intervention.
- Develop an individualized goal plan with every youth upon admission and regularly review and update the plan.
- Meet individually with youth on a regular basis to review progress, address barriers, and establish goals.
- Assist youth in developing short-term and long-term goals related to housing, education, employment, health, relationships, independent living, and personal development.
- Support youth in developing self-advocacy, decision-making, communication, problem-solving, and conflict-resolution skills.
- Monitor progress toward individual goals and document outcomes.
- Provide services using trauma-informed, culturally responsive, youth-centered, and LGBTQ+ affirming practices.
2. Safe House Referrals, Admissions, and Intake
- Review referrals for the Casa Q Safe House and determine whether referred youth meet program eligibility requirements.
- Assess whether Casa Q can appropriately and safely meet the needs of each referred youth.
- Participate in admission decisions in collaboration with the Program Director and/or other designated Casa Q staff.
- Communicate professionally and consistently with referral sources regarding referrals, eligibility, placement, and next steps.
- Communicate with case managers, schools, child welfare professionals, behavioral health providers, families/guardians when appropriate, and other referral partners.
- Request additional information or documentation when needed to make an informed admission decision.
- Schedule and conduct pre-admission interviews with prospective youth and appropriate referral partners.
- Conduct Safe House intake interviews and complete all required intake documentation.
- Explain Casa Q Safe House services, expectations, rights, responsibilities, rules, and available supports during the intake process.
- Identify immediate safety, behavioral, educational, medical, behavioral health, and other support needs during intake.
- Complete or coordinate initial safety planning following admission.
- Begin the youth's individualized goal plan following intake.
- Ensure all required assessments, releases of information, consents, and supporting documentation are completed and maintained in the client file.
- Coordinate with Client Advocates and other Safe House staff to ensure a smooth transition from referral and intake into the program.
- Follow up with referral sources and other members of the youth's support team as appropriate.
- Ensure admission and intake decisions and processes are consistent with Casa Q policies, licensing requirements, program requirements, and the safety and best interests of the youth.
3. Life Skills Development
- Develop, facilitate, and document individualized and group life-skills activities.
- Assist youth in developing skills necessary for successful independent living, including:
- Personal hygiene
- Budgeting and financial management
- Employment readiness
- Educational planning
- Cooking and nutrition
- Household management
- Personal organization
- Time management
- Communication
- Conflict resolution
- Self-advocacy
- Community navigation
- Public transportation
- Decision-making and problem-solving
- Identify areas where individual youth need additional support or skill development.
- Monitor and document progress toward life-skills goals.
- Collaborate with Client Advocates to reinforce life-skills development throughout the youth's stay.
4. Education and Employment Planning
- Assist youth in establishing educational goals and identifying appropriate educational opportunities.
- Support youth with school enrollment, GED programs, vocational programs, college, training programs, and other educational resources.
- Coordinate with schools, educational programs, and other appropriate providers.
- Assist youth with employment exploration, job applications, resumes, interviews, and workplace readiness.
- Help youth identify career interests and opportunities for long-term employment and financial stability.
- Track educational and employment progress and incorporate progress into the youth's individualized goal plan.
5. Safety Planning and Crisis Support
- Develop individualized safety plans with each youth.
- Review and update safety plans when circumstances, risks, or needs change.
- Help youth identify potential risks, protective factors, coping strategies, and supportive resources.
- Respond appropriately to safety concerns, behavioral concerns, and crisis situations.
- Collaborate with the Program Director and other staff regarding significant safety or behavioral concerns.
- Follow Casa Q policies and procedures regarding mandated reporting, incident reporting, crisis response, confidentiality, and youth safety.
- Support youth in developing strategies for managing conflict, stress, and difficult situations.
6. Care Coordination and Advocacy
- Coordinate services and referrals based on each youth's individual needs.
- Connect youth with behavioral health, medical, dental, educational, employment, housing, legal, transportation, and other community resources.
- Collaborate with behavioral health providers, medical providers, schools, child welfare professionals, probation/court personnel when applicable, family/support systems, and community partners.
- Follow up on referrals to ensure youth are successfully connected to services.
- Assist youth in navigating community systems and accessing available resources.
- Advocate for youth when barriers to services or resources arise.
- Maintain appropriate communication with service providers while protecting client confidentiality.
- Participate in multidisciplinary meetings and case consultations as needed.
7. Individualized Goal Planning
- Ensure every youth has a current and meaningful Individualized Goal Plan.
- Develop goals collaboratively with youth rather than creating goals on their behalf.
- Ensure goals are measurable, realistic, strengths-based, and connected to the youth's identified needs.
- Review goal plans regularly and update them as the youth's needs and circumstances change.
- Document progress, accomplishments, barriers, and next steps.
- Help youth identify and celebrate progress toward independence and self-sufficiency.
- Coordinate the work of the care team around each youth's identified goals.
8. Documentation and Client Files
- Maintain complete, accurate, timely, and organized client files.
- Complete DAP notes and other required case management documentation within established timelines.
- Ensure client files contain current assessments, goal plans, safety plans, intake documents, releases, referrals, and other required records.
- Document client progress, participation, barriers, significant events, referrals, and outcomes.
- Regularly review client files to ensure documentation is complete and current.
- Maintain confidentiality and comply with Casa Q policies, contractual requirements, licensing requirements, and applicable laws and regulations.
- Ensure documentation accurately reflects services provided and client progress.
9. Team Meetings and Staff Collaboration
- Facilitate and/or coordinate monthly team meetings regarding youth care, progress, goals, and case management needs.
- Prepare relevant case information and updates for team meetings.
- Ensure appropriate information is communicated among Case Managers, Client Advocates, supervisors, and other members of the care team.
- Participate in staff meetings, supervision, case consultations, trainings, and other required meetings.
- Collaborate with Client Advocates to ensure consistency and continuity of care across shifts.
- Help develop consistent approaches for addressing youth needs, behaviors, goals, and safety concerns.
- Identify program needs and recommend improvements to strengthen services provided to youth.
10. Safe House Program Support
- Maintain a consistent and visible presence within the Safe House.
- Engage directly with youth during scheduled work hours.
- Participate in youth activities and programming.
- Support a safe, respectful, affirming, and inclusive environment.
- Work collaboratively with Client Advocates and other Safe House staff.
- Address concerns related to youth behavior, relationships, house expectations, and conflict using trauma-informed approaches.
- Assist with identifying program improvements that strengthen the Safe House environment and youth experience.
SCHEDULE AND AVAILABILITY
This is a full-time Monday–Friday position with a flexible schedule based on the needs of the Safe House and youth served.
Because this position requires direct engagement with youth, the Case Manager must have regular evening availability. The schedule should include evening hours when youth are present and available for individual case management meetings, life-skills activities, educational planning, team collaboration, and other direct services.
The Case Manager may occasionally be required to adjust their schedule based on client needs, admissions, meetings, emergencies, or program activities.
The specific weekly schedule will be established in collaboration with the supervisor.
