Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our assisted living and memory care team. The ideal candidate will provide high-quality patient care in assisted living and memory care communities. This role requires a strong understanding of nursing practices, excellent communication skills, and the ability to work collaboratively with healthcare professionals to ensure optimal patient outcomes.
Duties
- Administer medications and treatments as prescribed by physicians.
- Conduct patient assessments and monitor vital signs.
- Assist in the development and implementation of patient care plans.
- Document patient information accurately in MAR systems.
- Collaborate with interdisciplinary teams to coordinate patient care.
- Educate patients and families on health management and disease prevention.
- Support patients with activities of daily living (ADLs) and mobility assistance.
- Ensure compliance with HIPAA regulations and infection control protocols.
Qualifications
- Valid LPN license in the state of practice.
- Experience in assisted living and memory care preferred.
- Experience working with diverse populations including individuals with developmental disabilities, Alzheimer’s care, memory care, and geriatrics is highly desirable.
- Strong skills in patient assessment.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.

Pay: $28.00 - $30.00 per hour
