Job Summary
Seeking full-time or part-time Physical Therapists to perform home health visits in the Weimar, TX and surrounding areas.
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized rehabilitation programs to help patients regain mobility, reduce pain, and improve their overall quality of life. Your expertise will support a diverse patient population across inpatient, outpatient, home health, and community settings. You will collaborate closely with multidisciplinary teams to deliver exceptional patient care, ensuring treatment plans are effective and aligned with best practices in physiotherapy. Responsibilities
- Conduct comprehensive assessments of patients’ physical conditions, including physiology and anatomy knowledge, to develop tailored care plans.
- Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and home & community care strategies.
- Monitor patient progress regularly and adjust treatment plans accordingly to optimize outcomes.
- Educate patients and their families on injury prevention, mobility exercises, and self-care techniques to promote long-term wellness.
- Document all patient interactions accurately within electronic medical record (EMR) systems and maintain detailed treatment records.
- Coordinate discharge planning and facilitate smooth transitions between inpatient care, outpatient services, home health visits, or community programs.
- Collaborate with healthcare professionals across disciplines to ensure comprehensive patient care and adherence to medical terminology standards.
Experience
- Proven experience in acute care settings such as hospitals or inpatient units is highly desirable.
- Demonstrated outpatient or clinic experience working with diverse populations including geriatrics and pediatrics.
- Familiarity with home health environments and community-based care programs enhances your ability to deliver holistic treatment.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques is essential for success in this role.
- Prior experience utilizing EMR systems for documentation and care coordination is preferred.
- Inpatient experience in hospital settings combined with outpatient or home & community care exposure will set you apart as a candidate.
- Additional certifications in trigger point therapy or specialized areas like orthopedics further strengthen your qualifications.

Pay: $51.02 - $61.45 per hour
