Community Liaison
Connected Home Health is seeking a dynamic and exceptional Community Liaison to support our growing mission of providing life changing service to our Home Health patients. The Community Liaison is an exciting opportunity to be a brand ambassador and culture champion with an established and amazing Home Health & Hospice team! The ideal candidate is professional, organized, home health and hospice savvy, compassionate, experienced working with a team, driven, proactive, quality focused, creative, fun (!), technologically adept, positive, and overall awesome.
- This position is Full-Time and benefited. The salary for this position is commensurate on experience and the range is $70k-$75k.

The Community Liaison will be responsible for:

Developing referral sources with agencies, hospitals, physicians, organizations

Relationship building with same

Identifying opportunities for formal or contractual relationships

Participation in community events
