Learn Beyond Healthcare LLC is seeking an experienced RN Case Manager / Primary Care Clinic RN for a 13-week contract assignment in Juneau, Alaska.
Responsibilities:
- Coordinate patient care and case management services.
- Perform clinic nursing assessments and patient education.
- Manage acute and chronic patient care needs.
- Coordinate referrals and multidisciplinary care.
- Document care using Meditech EMR.
Requirements:
- Active Alaska RN License – Required
- BLS Certification – Required
- Recent 1+ year of RN Case Management experience – Required
- Looking for travelers that live at least 50+ miles away from the facility – Required
