We are a compassionate and holistic medical group that is looking for an NP to provide exceptional care to our patient panels. You would be supported by a lead physician and care managers that help to ensure your plans are followed and minimize administrative work. We believe in deeply getting to know our patients and you would have a limited patient panel, scheduling autonomy (and the ability to do 5x8 or 4x10), no nights and weekends and a team that is building the future of health care at home through our integrated technology model.

About the Role
You'll provide comprehensive primary care to senior patients at assisted living facilities within 30 miles of Seattle. You'll typically work at one facility per day, supported by a dedicated care management team that knows your patients and handles the busywork—so you can focus on clinical judgment and relationships.
Your week: ~3–4 days in the field, ~1–2 days on telemedicine and care coordination (about 30–50% virtual/flexible work).
What You'll Do
- Conduct comprehensive assessments and build individualized care plans
- Collaborate with care managers, physicians, and facility teams
- Educate and support patients and families
- Monitor progress and adjust treatment plans
- Travel to facilities within 30 miles of Seattle
What You Bring
- Active WA Nurse Practitioner or Physician Assistant license
- Deep compassion and care for patients; willingness to work closely and collaborate with other members of the team.
- Clean driving record + reliable transportation
- Senior care / home health experience preferred (assisted living a big plus)
- Strong primary care and care management knowledge
- Excellent communication + ability to work independently and excellent clinical judgement
The Package
✅ $145,000–165,000 + $5-10,000 sign on bonus
