The Role
As a Hospice RN Case Manager, you'll coordinate and deliver comfort-focused nursing care to patients in their homes, nursing facilities, and assisted living communities. You'll serve as the case manager on an interdisciplinary team, managing a caseload with a focus on pain and symptom management, quality of life, and family support.
What You'll Do
- Perform comprehensive assessments of patients' physical, psychosocial, emotional, and spiritual needs
- Develop, implement, and update individualized hospice plans of care in coordination with the interdisciplinary group (IDG) and physician orders
- Provide skilled nursing care centered on pain and symptom management and end-of-life comfort
- Assess and document hospice eligibility and support certification/recertification of terminal illness
- Coordinate care across the IDG (social work, spiritual care, hospice aides, volunteers, bereavement)
- Provide supervisory visits for hospice aide and homemaker services per Medicare Conditions of Participation
- Educate patients, families, and caregivers on disease progression, medications, and comfort measures
- Document accurately and timely in the electronic medical record
- Participate in scheduled IDG meetings and an on-call rotation
What You'll Bring
- Current, active Ohio RN license in good standing
- Minimum 1 year of nursing experience (hospice, palliative, home health, or oncology preferred)
- Current BLS/CPR certification
- Valid Ohio driver's license, reliable transportation, and current auto insurance
- Strong assessment, documentation, and communication skills
- A compassionate, patient- and family-centered approach to end-of-life care

Schedule
- Full-time, Monday–Friday, with on-call rotation
- Travel to patient homes, nursing homes and assisted living
