Location: Dallas, TX (Addison)
The Nurse Practitioner (NP) provides comprehensive hospice care to patients in their homes, including symptom management, care plan development, recertification visits, and face-to-face encounters. The Nurse Practitioner works closely with physicians and the Interdisciplinary Team (IDT) to monitor patient progress, ensure accurate documentation, and support quality end-of-life care. This role focuses on patient assessments, clinical documentation, hospice recertifications, and coordination of care to ensure patients receive appropriate hospice services.
This position requires strong clinical assessment skills, hospice experience, and the ability to work independently while collaborating with the hospice care team.
Licensed Nurse Practitioner per state requirements
Minimum 2 years of experience as a Nurse Practitioner
Minimum 2 years of hospice experience required
Strong clinical assessment, documentation, and communication skills
Experience with hospice recertifications and face-to-face encounters preferred
Experience working with an interdisciplinary healthcare team
Proficiency with electronic health records (EHR) systems
Reliable transportation, valid driver’s license, and current auto liability insurance
Ability to read, write, and communicate effectively in English
Perform patient assessments and gather clinical information to support diagnosis, prognosis, and hospice recertification
Complete face-to-face encounters and documentation required for hospice recertification
Develop, implement, and revise hospice plans of care in collaboration with physicians and the Interdisciplinary Team (IDT)
Provide symptom management, pain management, and clinical support to hospice patients
Provide education and support to patients, families, and caregivers regarding disease process and end-of-life care
Monitor patient progress and coordinate care with nurses, social workers, aides, and physicians
Communicate changes in patient condition to physicians and Interdisciplinary Team members
Complete clinical documentation accurately and in a timely manner in the electronic medical record
Participate in IDT meetings, education programs, and quality improvement initiatives
Participate in on-call rotation as required
Ensure compliance with hospice regulations, infection control practices, and patient rights
Support quality assurance initiatives and hospice program standards
