Job Overview
Performs ADL duties for individuals including but not limited to; bathing, dressing, toileting, grooming, preparing meals, and lifting or transferring to/from various surfaces. Performs general housekeeping duties to maintain the cleanliness and safety of the residence. Administers medications and topicals according to doctors’ orders and under the direction of a nurse. Implements individualized program plans and maintains records and charts as needed. Accompanies individuals in community inclusionary activities, medical appointments and all other applicable activities.
Responsibilities
- Assist individuals with activities of daily living (ADL’s) including but not limited to; toileting, changing adult diapers, lifting/transferring, showering, meal programs, etc.
- Accompanies persons supported to medical appointments and recreational activities in the community.
- Collaborate with clinical team to create opportunities for inclusive care with persons supported.
- Administers medications and treatments as prescribed by clinical teams (training provided).
- Prepares and services meals for persons supported following prescribed/approved menus.
- Maintains cleanliness of environment by performing general housekeeping and laundry.
- Performs other duties as assigned by the leadership team.
Qualifications:
To be successful in this role, candidates must have a high school diploma, and most be able to communicate effectively in English. A minimum of one (1) year experience working in a healthcare setting is required. Candidates must be flexible to float to various locations within their chosen borough and be able to work overtime when needed. Candidates must have the ability to push, pull, lift, stand for long periods, ambulate and bend to ensure safety.
