We are seeking an empathetic, self-motivated, and organized Client Care Coordinator to join our team in Lubbock, Texas. In this role, you will be the face of Comfort Keepers for prospective and existing clients, driving home care consultations, developing individualized care plans, and building lasting relationships within our community.
The Client Care Coordinator (CCC) manages the complete client experience from initial referral to continuity of care:
- Field Outreach & Relationship Building: Actively build and maintain strong relationships with key referral sources in the Lubbock community, including assisted/independent living facilities, hospice providers, geriatric care managers, medical specialists, discharge planners, and other senior-focused organizations.
- Inquiry, Consultation & Onboarding: Respond to referral calls, educate prospective clients and families on our services, and conduct compassionate, in-home consultations to assess needs. Guide families through the intake and onboarding process, ensuring all care documentation and emergency procedures are established before care begins.
- Ongoing Care Plan & Quality Management: Perform regular client check-ins, home visits, and quality assurance reviews. Continuously evaluate evolving health conditions to update customized care plans and ensure uninterrupted, high-quality care through coordination with family resources, community providers, insurance companies, case managers, and third-party payors.
- Caregiver Matching & Scheduling Collaboration: Work directly with the Scheduling team to match caregivers to clients based on skills, personality, and care needs, ensuring that client shifts align with caregiver qualifications to deliver a platinum experience.
- Client Support & Resolution: Serve as the primary, trusted point of contact for clients and their families to address questions, resolve concerns, and adjust care approaches or service hours as needed.
Job Requirements

- Multitasking & Organization: Ability to manage multiple tasks, prioritize responsibilities, meet deadlines, maintain meticulous attention to detail, and switch between activities efficiently without compromising quality.
- Education & Experience: Minimum of a High School Diploma or GED; minimum of 2 years of related experience in healthcare, home care, social work, senior living, or client management.
- Technical Proficiency: Strong computer literacy, including proficiency in Microsoft 365 (Word, Excel, PowerPoint), DocuSign, and scheduling/CRM platforms (experience with VIV or similar systems is a plus).
- Interpersonal & Communication: Exceptional customer service, empathetic bedside manner, outstanding people management skills, and strong written and verbal communication abilities.
- Relationship Building: Proven ability to build positive, lasting relationships with clients, families, healthcare professionals, and individuals from diverse backgrounds.
- Reliability & Requirements: A reliable vehicle, valid driver’s license, auto insurance, and the ability to pass a comprehensive background check (including motor vehicle records check) and drug screen.
- Professionalism & On-Call: Ability to manage personal stress, consistently present a positive and professional image, and participate in an assigned on-call rotation for after-hours support.
- Other Duties: Perform additional operational, field, or administrative duties as assigned.

Pay: $32,000.00 - $42,000.00 per year
