Provenir Healthcare is partnering with a well-established outpatient practice in Haines City, FL that is seeking a Board-Certified Internal Medicine or Family Medicine Physician to join its growing team. This opportunity offers the chance to care exclusively for the senior population in a collaborative, patient-centered, value-based care environment focused on preventive medicine, wellness, and delivering high-quality outcomes.
This position offers:
- 100% Outpatient schedule
- Clinic hours: Monday-Friday 8a-5p
- EMR: eClinicialWorks

RESPONSIBILITIES:
- Providing adult primary care focusing on the Senior/Medicare Advantage patient
- Comfortable with a small panel size and building relationships with your patients
- Average 15 patients/day
- Able to work with a Team who focus on Value Based Care
QUALIFICATIONS:
- Active state medical license-MD or DO
- Board certified/eligible Internal or Family Medicine
- Value-based care experience preferred
- Geriatric Fellows welcome
