Job Overview
We are seeking compassionate and dedicated Caregivers/Home Health Aids to provide exceptional in-home and community-based care to individuals of all ages. In this vital role, you will support clients with daily activities, medical needs, and emotional well-being, fostering independence and enhancing quality of life. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions, recording vital signs and changes in health status accurately
- Administer medications following prescribed schedules and document medication administration in medical records or electronic health record (EHR) systems
- Support clients with specialized needs such as Alzheimer’s care, dementia care, hospice care, or working with individuals with developmental disabilities
- Operate medical equipment like Hoyer lifts, catheters, and other assistive devices safely and effectively
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation
- Document all patient interactions and medical information in compliance with HIPAA regulations using EMR (Electronic Medical Records) or EHR systems
Requirements
- Prior experience in home health care, assisted living, nursing homes, or long-term care settings preferred
- Knowledge of medical terminology, medication administration, and patient care procedures
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice or dementia training are a plus
- Ability to work independently while following detailed care plans and protocols
- Strong communication skills to collaborate effectively with clients, families, and healthcare teams
- Experience working with individuals with disabilities or special needs highly valued
- Valid driver’s license and reliable transportation preferred for home visits
