Responsibilities
- Deliver high-quality nursing care to residents with memory impairments
- Conduct comprehensive patient assessments, monitor vital signs, and evaluate symptoms
- Implement individualized care plans focusing on dementia and Alzheimer’s care, ensuring safety and comfort at all times.
- Manage medication administration accurately using EMR (Electronic Medical Records) systems likemaintaining precise medical documentation.
- Assist with activities of daily living (ADLs), including catheter care, sterile procedures, and physical examinations, while adhering to HIPAA regulations for patient privacy.
- Collaborate with interdisciplinary teams on discharge planning, case management, and triage to optimize patient outcomes.
- Support residents’ emotional and physical needs through compassionate communication and behavioral health interventions tailored for those with developmental disabilities or behavioral challenges.
Requirements
- Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) preferred.
- Proven experience in long-term care facilities, assisted living communities, or hospital settings—particularly with dementia or Alzheimer’s patients.
- Ability to perform complex patient assessments including vital signs
