Position Overview
As a Physical Therapist (PT), you will evaluate, develop, and implement individualized, evidence-based treatment programs designed to improve residents' mobility, strength, balance, endurance, and overall functional independence. You will play a vital role in interdisciplinary collaboration, resident and caregiver education, and achieving exceptional clinical outcomes.
Key Responsibilities
- Evaluate residents to assess mobility limitations, strength deficits, balance impairments, pain, and functional challenges
- Develop and implement individualized treatment plans focused on mobility, gait training, fall prevention, strengthening, endurance, and pain management
- Monitor resident progress and modify treatment plans to maximize functional outcomes
- Supervise and collaborate with Physical Therapy Assistants (PTAs) to ensure high-quality patient care
- Recommend assistive devices, adaptive equipment, and environmental modifications to promote safety and independence
- Educate residents, families, and caregivers on therapeutic exercises, mobility strategies, and discharge recommendations
- Maintain accurate and timely documentation in accordance with state, federal, and company requirements
- Participate in interdisciplinary team meetings, care plan conferences, and resident-centered care initiatives
- Ensure compliance with all regulatory standards, professional ethics, and facility policies
Qualifications
- Active Texas Physical Therapist (PT) license
- In good standing with all applicable licensing boards and regulatory agencies
- Experience in long-term care, skilled nursing, geriatric rehabilitation, or post-acute care preferred
- Familiarity with Net Health documentation software is a plus
- Strong clinical assessment, communication, leadership, and problem-solving skills
- Passion for helping older adults achieve their highest level of function and independence
