Responsibilities
- Provide direct patient care, including administering medications, performing basic life support, and monitoring vital signs.
- Assist in the development and implementation of individualized care plans based on patient assessments and medical histories.
- Collaborate with registered nurses and physicians to coordinate patient care and ensure adherence to treatment protocols.
- Maintain accurate medical documentation in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.
- Perform procedures such as IV insertion, venipuncture, catheterization, and specimen collection as required.
- Educate patients and their families on health management strategies, medication administration, and post-discharge care.
- Participate in triage processes to prioritize patient needs effectively in fast-paced environments such as emergency rooms or urgent care settings.
- Uphold infection control standards while providing compassionate care to patients across various settings including long-term care facilities, hospitals, and outpatient clinics.
Qualifications
- Valid LPN license with proven experience in acute care settings such as hospitals or nursing homes.
- Familiarity with medical terminology, ICD-9/ICD-10 coding systems, and electronic medical record (EMR) systems like Cerner or Athenahealth.
- Experience working with diverse populations including seniors, individuals with developmental disabilities, and patients requiring memory care or hospice services.
- Strong clinical skills in areas such as pain management, wound care, and medication administration.
- Excellent communication skills for effective interaction with patients, families, and interdisciplinary teams.
- Knowledge of critical care practices is preferred; experience in Level I or Level II trauma centers is a plus.
- Ability to perform physical examinations and conduct diagnostic evaluations as part of comprehensive patient assessments.
