LPN Licensed Practical Nurse

Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments as needed
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Monitor and report changes in patient condition to the registered nurse or other healthcare professionals
Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery
Participate in quality improvement initiatives to enhance patient outcomes and resident satisfaction
