What You'll Be DoingDeliver Exceptional Rehabilitation Services
- Implement treatment plans developed by licensed Physical Therapists
- Provide gait training, balance activities, ROM exercises, and mobility interventions
- Teach exercises, safety strategies, and self-care techniques to patients and caregivers
- Monitor patient progress and communicate updates to the supervising PT
- Help patients maximize their independence and quality of life
Collaborate Across The Continuum
- Work alongside Physical Therapists, nurses, and interdisciplinary clinicians
- Document visits and patient progress using streamlined EMR systems
- Contribute to outstanding patient outcomes
- Support patients throughout their recovery journey
Required Qualifications
- Active Minnesota Physical Therapy Assistant license
- Valid driver's license, insurance, and reliable transportation
- Strong communication and relationship-building skills
- Passion for patient-centered care
Preferred Qualifications
- Home health experience
- Experience with geriatric or medically complex patients
- Desire to work independently while being part of a collaborative team
