REPORTING: The Physical Therapist reports to the Director of Rehabilitation
JOB DUTIES:

Basic knowledge of physical medicine, physical therapy modalities, anatomy and physiology.

Basic knowledge of physical therapy machine operation, maintenance and repairs of same.

Provides assessment, treatment, and rehabilitative services to patients who have experienced
- injury or disability disease. Provides residents with exercises.

Adheres to standards of physical medicine practice.

Ability to observe and evaluate treatment effect, discusses observations with staff, Physical
- Therapy Assistant, Physician, and Director of Rehabilitation.

Responsible for the monitoring and documentation of resident treatment and response.

Assists with formulation of a teaching plan based upon identified learning needs and assesses effectiveness of learning, family is included in teaching as appropriate, staff and physician is apprised of conclusions.

Meets current documentation standards and policies.

Adheres to facility’s Policy and Procedure.
REQUIREMENTS:
1. CPR Certification (required).
2. Licensed Physical Therapy Therapist (required).
3. 1-2 years experience in long term care preferred.
4. Good physical and mental health.
5. Must be able to perform any physical requirement of the position which includes lifting dead weight or unassisted weight of 50 pounds.
6. Is capable of following oral and written instructions.
7. Have training, knowledge and experience which allows for general supervision of responsibilities.

Rehab – SLP
REPORTING: The Speech Language Pathologist reports to the Director of Rehabilitation.
JOB DUTIES:

Basic knowledge of physical medicine, speech therapy modalities, anatomy and physiology.

Treat patient in accordance with Speech Language Pathology Standard of Care and Practice.

Provides assessment, treatment, and rehabilitative services to patients who have experienced injury or disability disease. Provides residents with exercises related to speech language pathology.

Adheres to standards of physical medicine practice.

Ability to observe and evaluate treatment effect, discusses observations with staff, physician and Director of Rehabilitation.
