Coral Springs Rehab and Nursing in beautiful Wilmington, DE is currently hiring for a full time Social Services Assistant.
This is a Monday-Friday position for the 8a-4:30p shift. There is a MOD rotation required for weekends as well.
Job Duties and Responsibilities:
Assist in planning, developing, organizing, implementing, evaluating and directing the social service programs of the facility.
Participate in discharge planning. development and implementation of social care plans and resident assessments.
Interview residents and families to obtain social history.
Ensure that all charted progress notes are informative and descriptive of services provided to residents.
