Overview:
Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members.

Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
- Provides effective instruction to the patient and family through standardized educational programs and/or individualized training.
- Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes. Productive and efficient in daily operations.
- Selects and performs the appropriate assessment procedures.
- Establishes and updates formal plan of care to address individual patient needs.
