Assists with providing pericare and ADL assistance with Clients (ADL is defined as Activities of Daily Living).

Assists with providing direct care and support for clients.

Properly reports any potential skin and medical concerns directly to the supervisor and program nurse.

Provides support and instruction to individuals in activities designed for developing good self-care, work readiness skills, socialization, behavior skills, and/or instruction and supervising individuals in actual work.

Required to report all incidents involving aggression, behavioral intervention, medical situations or treatments relating to staff or individuals, any pertinent information to the program supervisor in a timely manner.

Updates information on each individual to keep it up to date.

Adheres to and supports all Vista Care policies and procedures.

Adheres to and supports all Vista Care core Values, actively supporting the Mission and Vision of the organization.

Responsible for completing daily charting, logs, documentation, and plans for individuals supported in a timely, thorough manner based upon all Vista Care policies and procedures.

Understands and able to identify signs of agitation for the purpose of implementing recommended behavior support(s).

Actively participates in inservice training and staff meetings
