Job Overview
Mental Health Case Manager to coordinate and deliver comprehensive mental health services within a residential setting. The case manager is considered a resource in that they should be familiar with community resources and service to which they may help their clients gain access.
Responsibilities
- Provide case management services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, programs, vocational services and opportunities, community integration, entitlements etc.)
- Provide individual counseling and support services; responsible for all documentation.
- Housing services including assistance with maintaining their voucher, education assistance with the life skills needed to maintain an apartment and resolving maintenance issues.
- Provide unit visits weekly and unit inspections monthly.
- Participate in resolution of landlord and lease issues and coordination with public housing authority recertification.
- Complete all necessary documentation to ensure compliance with funding requirements and agency quality assurance standards.
- Attendance at weekly supervision and staff meetings.
Requirements
- Bachelor’s Degree in Psychology, Social Work or other related mental health discipline.
- 2 years experience working with persons with histories of substance abuse and/or mental illness.
- Valid driver’s license required.
- Ability to travel as needed
Pay: $50,000.00 per year
