To contract with us for referrals, you will need a copy of the following documents:
- Level 2 background screening - https://info.flclearinghouse.com
- Local background check from Police Department or Sheriff’s Office
- C.N.A. License or HHA Certificate Or LPN / RN
- Driver License & Social Security Card (or Federal Tax ID)
- Auto registration & proof of Auto Insurance
- Professional Liability Insurance
- CPR certification
- Physical examination, within the past 6 months, to show free and clear of Communicable Diseases.
- First Aid Course (live not online) approved by Agency for Persons with Disabilities (APD)
- High School Diploma or GED (for APD cases only)
- Voided check or equivalent
- CEU certificates
