RN – Case Manager
Location: Cortlandt Manor, NY 10567
13-Week Travel Contract
Start Date: 2–3 Weeks After Offer
Travel Pay Package: $2,880/week = $28/hr on W2 + $1,760/week Per Diem
Local pay: $2,520/weekly =$63/hr on w2
$500 Bonus Available
Schedule
- 40 Hours per Week
- Preferred Schedule:
- 4x10-Hour Shifts
- 7:00 AM – 5:00 PM or 8:00 AM – 6:00 PM
- Alternative Schedule Available:
- 5x8-Hour Shifts
- 8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM
- Every Other Sunday
- Two Weekends Per Month Required
Position Overview
We're seeking an experienced RN Case Manager to coordinate patient care, discharge planning, and resource utilization within a collaborative healthcare environment. This position offers the opportunity to work with a leading healthcare system while ensuring patients receive efficient, high-quality care throughout their hospital stay.
Required Qualifications
- Active New York RN License (Required at Submission)
- BSN Required
- Minimum 3 Years of Recent Case Management Experience
- Strong Discharge Planning (DCP) Experience Required
Key Responsibilities
- Coordinate patient care plans across the continuum of care
- Facilitate discharge planning and transitions of care
- Collaborate with physicians, nursing staff, social workers, and interdisciplinary teams
- Ensure appropriate utilization of healthcare resources
- Assess patient needs and identify barriers to discharge
- Advocate for patients and families to achieve optimal outcomes
- Maintain accurate documentation and case management records
Pay: $2,200.00 - $2,800.00 per week
