Position Summary
True Self Recovery is seeking a compassionate, highly organized Case Manager to join our Rogers, AR team. The Case Manager plays a critical role in bridging clinical care and real-world logistics — coordinating with clinical staff to ensure clients have the appointments, documentation, and resources they need both during treatment and as they transition into life after treatment.
This role is ideal for someone who thrives on organization, communicates well across teams, and is passionate about removing barriers that stand between clients and lasting recovery.
Key Responsibilities
- Coordinate closely with clinical staff (counselors, Lead Counselor, and clinical leadership) to identify and prioritize each client's case management needs
- Schedule and track medical/doctor appointments for clients
- Schedule and coordinate legal appointments as needed (court dates, attorney meetings, legal aid referrals)
- Assist clients in obtaining Social Security cards and state-issued IDs, including gathering required documentation and accompanying clients to appointments when necessary
- Identify and secure funding sources for sober living placements
- Research and coordinate post-treatment housing/living arrangements
- Maintain accurate, up-to-date client records and appointment logs in KIPU
- Communicate appointment schedules and case updates clearly with clinical staff to ensure continuity of care
- Build relationships with community resources, sober living homes, legal aid organizations, and government agencies (e.g., Social Security Administration, DMV) to streamline referrals
- Advocate for clients' needs and follow up to ensure appointments and applications are completed
- Maintain compliance with confidentiality requirements (HIPAA/42 CFR Part 2) and facility documentation standards
Qualifications
- High school diploma or equivalent required; Associate's or Bachelor's degree in social work, human services, psychology, or related field preferred
- Prior experience in case management, social services, or a substance use/behavioral health treatment setting strongly preferred
- Working knowledge of community resources in Northwest Arkansas (legal aid, Social Security Administration, sober living providers, funding/grant programs) a plus
- Strong organizational skills with the ability to manage multiple client schedules and deadlines simultaneously
- Excellent communication skills, both with clients and across the clinical team
- Comfortable working in KIPU or willing to learn
- Compassionate, non-judgmental approach to working with individuals in early recovery
- Reliable transportation (may require accompanying clients to appointments)
