Join a Growing Patient-Centered Home Physical Therapy Practice
We are seeking a compassionate, motivated, and skilled Physical Therapist to provide one-on-one care in patients' homes. This position offers flexibility, autonomy, and the opportunity to make a meaningful impact on patients' lives while helping them recover safely in their own environment.
Position Overview
The Physical Therapist will evaluate and treat patients in their homes, develop individualized treatment plans, and collaborate with physicians, caregivers, and other healthcare professionals to optimize patient outcomes.
Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop and implement individualized treatment plans.
- Provide skilled therapeutic exercise, manual therapy, gait training, balance training, and functional mobility interventions.
- Educate patients and caregivers regarding diagnoses, treatment plans, home exercise programs, and safety recommendations.
- Complete accurate and timely documentation in the electronic medical record.
- Communicate effectively with referring physicians and healthcare team members regarding patient progress and plan of care.
- Maintain compliance with Medicare, insurance, and professional practice standards.
- Travel to patient homes within the assigned service area.
QualificationsRequired
- Doctor of Physical Therapy (DPT) or equivalent degree.
- Current New York State Physical Therapy License.
- Valid driver's license and reliable transportation.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
Preferred
- Home health, outpatient orthopedic, geriatric, or sports medicine experience.
- Knowledge of Medicare documentation requirements.
- Experience with electronic medical records.
